Evaluation of Simulation Performance
Participation in the Simulation Lab is the same as participation in the laboratory and clinical settings. You are evaluated individually at the end of your simulation experience based on your ability to meet the objectives of the day. You will always be made aware of Simulation Center objectives for your group so there is never a surprise as to what you need to gain from the experience.
Formative Evaluation
Each time you come to the Simulation Center, you will bring a Simulation Checklist. This can be found under the Documents tab. This checklist is designed as a tool for you to know what is expected of you for each
scenario/simulation. You may look at this before the simulation begins and then it must be handed to your
Evaluating Instructor. This checklist will be used to give you feedback to take with you when you leave the
Simulation Room.
Any concerns with the evaluation should be discussed with the Simulation Instructor at the time of the evaluation and you will be able to take this evaluation with you when you leave so you can learn for your next experience.

Summative Evaluation
There is a separate evaluation tool to evaluate your performance in the Simulation Center and provides a summary of where you stand in six Core Values. This evaluation will be shown and discussed with you. The goal of the evaluation is for you to work towards an independent thinker and practitioner. The original copy of this evaluation will be kept in the Simulation Center to be used throughout your simulation experiences at Phoenix College. At the end of your program it will then be placed in your permanent file.
It is a goal that your Summative Evaluation be reviewed with you before you leave from your Simulation Day. There may be times however, that this will not be possible. You may be asked to return to review this evaluation at a later date or time. If there is an unforeseen circumstance that needs to be reviewed and/or remediated, the Simulation Insturctors reserve the right to contact you after your Simulation Day to discuss. Your Block Coordinator may need to be contacted and a plan of action may need to be drafted. The lines of communication need to remain open so please do not hesitate to contact your Simulation Instructor via email.

Conference Form Documentation
Any unsafe practices observed in the Simulation Lab will be documented on a Conference Form, shared with your clinical instructor and/or Block Faculty and kept in your permanent file. This Conference Form is a means of documentation. Your clinical instructor is notified so you can be remediated and checked off as ‘safe’ in the clinical setting. Please refer to your MaricopaNursing Handbook regarding unsafe practices and progression through the program.
